2 Proof of Residence - Current Resident
The proof of residence must be in the name of the person with legal responsibility for the child
Current Utility Bill (water, sewer, gas, electric showing both the service and mailing address)
Current Property Tax Statement (with Homestead Exemption)
Lease Agreement
Student(s) must be listed as an occupant on apartment lease.
An agreement between individuals requires both signatures to be notarized.
2 Proof of Residence - Non-Resident
If you currently reside with a district resident and do not have a utility bill in your name:
Affidavit of Address -Please click on link to print Affidavit. It must be notarized.
1 proof of address:
DL or State ID with current address listed
Documentation from Employer, such as pay check stub, personnel papers, tax forms, or withholding statements
Insurance papers issued in the name of the enrolling party
Bill and/or statement form bank, or major credit card company.
Approved District Transfer Paperwork -
*Transfers are approved at the campus level according to class size. If you have not already, you will need to fill out the application for transfer and return it to the campus principal to be processed first. Upon approval you will then complete this process.
Immunization Record –
A record showing that the child has the immunizations required by Education Code 38.001
Affidavit issued by the state exempting the student from being fully vaccinated
Homeschool Documents-
*Please note that the parent/guardian is responsible for getting all Homeschool records to the school*
School Documents- Woden ISD can request these from other public schools, but providing them with expedite the enrollment process.
Information about any special programs the student is in. Including IEP or 504.
Incoming K-9th graders must have a Report Card showing promotion or placement in the next grade.
10th through 12th graders must have a Transcript for prior high school years.
International high school students – You will need to email the Registrar’s office the following:
6th, 7th, and 8th grade report cards
9th through 12th grade transcripts
International Jr High and Elementary students - You will need to email/ provide your last report cards showing completion or promoted to the next grade.
